I think that whoever wrote that text must have been thinking of the sentence "death by a thousand cuts" which is an old form of execution in China (you can read a detailed explanation here http://www.answers.com/topic/death-by-a-thousand-cuts ) .

The sentence has been used in various contexts and here I they are applying it to errors. It is not one big mistake that gets you but lots of little ones.
