problemy z oddechem. Po ustaleniu badaniem echokardiograficznym, że IVC nie jest poszerzona, okazało się, że jest właśnie "niepodatna oddechowo" i podane jest obliczenie RVSP - 48mmHg.

Generalnie chodzi o to, że żyła nie porusza się przy wykonywaniu oddechu.
Po analizie wyników badanej grupy u 88% pacjentów stwierdzono prawidłową średnicę żyły głównej dolnej, u 10% - poszerzoną żyłę główna dolną z zachowaną zmiennością oddechową, a 76 osób (2%)- poszerzoną i nieruchomą oddechowo.

The right atrial pressure (RAP) can be estimated from the end-expiratory diameter and respiratory changes of the inferior vena cava (IVC) as follows (figure 3)[2]:
• Normal diameter (≤2,1 cm) and inspiratory collapse (>50%), RAP 3 mmHg (range, 0-5 mmHg)
• Dilated IVC (≥ 2,1 cm) with reduced respiratory variation (<50%), RAP 15 mmHg (range, 10-20 mmHg)
• IVC diameter and respiratory variation unfitting the previous paradigm, without resolving hepatic flow pattern (absence of prevalent systolic or diastolic flow component) or tricuspid E/e’ wave ratio, RAP 8 mmHg (range, 5-10 mmHg)

In spontaneously breathing patients: The IVC diameter and respiratory variation indirectly reflects the RA pressure. For example, a dilated IVC (>2 cm) without the normal decrease in diameter (>50% with sniffing) usually indicates an elevated RA pressure.
In patients on mechanical ventilation: The IVC diameter and respiratory variation can help you predict if the patient will respond to a fluid challenge. For example, a hypotensive patient with a very small collapsed IVC (< 1cm) will probably responds to intravenous fluid challenge.
