Les micro-fractures
Steadman a décrit cette technique en 1994. Les lésions doivent être dans un premier temps débridées de façon à s'affranchir de tout fragment cartilagineux puis les perforations sont réalisées avec un poinçon dont il existe plusieurs angulations. L'utilisation d'un poinçon repose sur l'idée d'éviter tout dommage thermique à l'os sous-chondral. Une mobilisation immédiate est instituée sur arthromoteur. La reprise de l'appui est effectué 8 semaines après l'intervention.
