Can anybody explain what this means (and maybe suggest a translation):

Gestion intégrée de la santé des plantes 
Il s’agit de fédérer des recherches pluridisciplinaires dans le domaine de la santé des plantes et plus particulièrement entre domaines bio-écologiques et socio-économiques en développant des référentiels partagés, en concevant et évaluant des systèmes de culture alternatifs, en identifiant des **leviers de rupture** pour la santé des plantes et en accompagnant le changement.

I think that probably in one way or other this refers to the conditions of cultivation, that is, the maximum (or minimum?) requirements the plants need to grow successfully.

Or ways of bringing about change, or anything along those lines - it actually goes on to refer to "en accompagnant le changement". Several of the translations of "rupture" in my reference use "change".

how to leverage disruptive innovation

Une idée peut-être un peu trop forcée :
concevoir ... systèmes ...alternatifs et accompagner le changement impliquent l'idée d'innovation
et on parle de "disruptive innovation" (en francais : innovation de rupture)

+ je vois qu'on utilise souvent le verbe to leverage dans ce contexte : to leverage disruptive innovation
