It sounds like it is how the post-graduate course fits in in the institution; what it relation is with the rest of the things they do... but I cannot think of a way to express this in a title... I'll keep thinking.

I'm not sure, but it may refer to the extent to which the graduate program (or certificate) is aligned with the university's (or medical school's) institutional outcomes, I'd imagine.

Institutional insertion

The literal translation is definitely a thing, and in the education sphere, I found this: "The article describes the ways in which the scholarship holders, the Program and its institutional agents mentors and authorities of the Civil Society Organizations implied in the Program-become acquainted. Likewise, based on these linkages and the institutional insertion referred by the fellows, we propose a typology..."
Although the use of "imply" there makes it look like a machine translation, it might work. Otherwise, things like Marcelo's suggestion should do the trick.
