English Medical Medical: Instruments Hernia repair
Full text is: For direct hernia, the defect is circumscribed at its base, the contents fully reduced, and the preperitoneal space is entered

I don't clearly understand, can it be reworded as: For direct hernia, the defect (hole) should be cleared to its bottom with the migrated organs (the intestines) is fully pushed to the inside, and then get access (by an incision) to the preperitoneal space.

Can anybody help me confirm or explain, any insight into this is much appreciated. Thanks a lot.

The defect here is not a hole. They probably mean the portion of incompetent abdominal wall tissues (posterior wall of inguinal canal) by saying "defect" here. So they do a cut at one side of the defect (not a direct cut, but kind of "around it"), then put any contents of the hernia sack (usually omentum, often intestines) back to abdominal cavity.
