CONTEXT? OK, standing alone... but what is the overall context? What is this document about? Is this an elderly person, perhaps? I wondered if the writer (doctor, other healthcare professional, ... ?) was trying to say that the condition of this person meant they are very close to needing to go into a nursing home, for example.
