(acoustic) window quality
Many patients will have suboptimal or in some cases minimal acoustic windows for ultrasound examination and this has been a source of much frustration for technologists and cardiology fellows.
For example, patients who are obese, those who have chronic lung disease, who are imaged supine or on a ventilator, or those who are recently post-op from cardiac or thoracic surgery will often have limited windows and possibly uninterpretable images no matter how skilled the sonographer

In patients without major cardiac geometry distortions, the American Society of Echocardiography (ASE) recommends a formula to estimate LVM from linear dimensions based on the assumption of the left ventricle as a prolate ellipsoid of revolution (Figure 1). Linear measurements of inter-ventricular septum wall thickness (IVST), as well as left ventricular internal diameter (LVID) and posterior wall thickness (PWT), should be done from the parasternal acoustic window in end diastole at the level of the LV minor axis (mitral valve leaflet tips) using 2D-targeted M-mode or directly from 2D images.

Acoustic window
This is from "my place"...totally "cool", the "acoustics" are great!

“As a result, the ultrasound beams from the transducer head must be aimed through acoustic windows in the thorax to image the heart. There are four typical acoustic windows; the parasternal, apical, subcostal, and suprasternal notch (Figure 1). The parasternal window is typically used for 2 chamber, long-axis views and short axis views of the heart. The apical window is typically used for the standard 4 chamber, long-axis view. The subcostal window is particularly useful for patients with lung disease or recent open-heart surgery and provides good views of major vascular connections to the heart and the interatrial septum. The suprasternal window is valuable for aortic root imaging.”
