I am not sure about the word "von" here.

Does it mean, " (Name) born on 5th April 1982 of Ramsen (Schaffausen) " or "(Name) born on 5th April 1982 in Ramsen (Schaffhausen)"

In earlier days in the UK until I think 1837, they used to put 'of [place name]' on BMD certs (birth-marriage-death, mainly the first and last) or wills because they did not check against the original birth certs of either parents or the person who died and they defined a person as with that name plus the place he was from to distinguish from others. So, if Swtzerland still works this way, they will put 'von' as an addition to the name. So 'of' would also be an option in English, although it would sound quite old-fashioned. Be careful though with 'born in', because it may just be that this person was not born there but is at this moment living there. It depends what Switzerland takes as a reference in this case.

Kommafehler in einem offiziellen Dokument? Vielleicht ist das Schweizerdeutsch und entspricht dem hochdeutschen "geboren am ... in ..."? Wenn mit Komma, würde ich auch eher "aus ..." sagen, obwohl das für so ein Zertifikat nicht so recht passen will.

I dont think "von" is a mistake. I have 2 more certificates given by different institute with the same format.
